That is mostly for caravans and in extreme circumstances can affect what weapons/armour the pawn can carry (worn/equipped gear does count against it but is rarely heavy enough to matter).

Items on the colony map itself is based on another stat and is usually 75 items (one stack) but can be exceeded e.g. butcher an elephant and the bill tells them to take to a stockpile then they will carry all the meat which is usually 2-3 stacks (140 items or more).

It's basically the difference between how much stuff you can just pick up and carry in your arms compared to how much you can carry if you pack it up properly.
Like, if you just go grab a pile of clothes you'll be able to grab a few pounds of it maybe, due to its bulk, but if you pack stuff up properly you can carry a lot more than that.

manipulation is the stat that effects your pawns ability to carry stuff on the map, so if he has some scars or medical conditions that effect that stat then he is not going to carry as much as he could if he was perfectly healthy...

is there like a stack size as well ? ie they can only pick up a number of a item ?

This is not true for vanilla rimworld. Some mods such as RPG stats change how carry weight works but normally there is no way to increase or decrease carry weight from 35kg for a pawn.

No. They can technically carry a virtually unlimited number of items and separate stacks. At the very least, even in ridiculous situations you will not hit any kind of limit even when they are hauling hundreds of separate item stacks.

It affects the stack size they can carry which I think is what jfoytek meant?

When at a location (like, in the map that your base is in) most items have a limit of 75 to carry at once, or a full stack, whichever is smaller. On the world map the stuff they carry can be in any amount, in any combination, so long as the total mass (including their weapon and clothing) is below 35kg.
