*usually* caused by being indoors, and not in range of a light, but really all you know is that it has been around too long.
Note that plant can die of old age, even if it never reached 100% growth.
Maybe you had severe temperature problems, resulting in ridiculously slow growth?
Maybe the plant is a legacy one from before you converted it into a covered garden?

Think you growing enough for 5 colonists there?
Also you don't need to assign a cut plant order on your crops, once they reach 100% grown your growers will automatically harvest them, if they have their priorities set correctly.
Well, I have multiple plants, but only one of them is modded (wheat), I set them to harvest because most of them started to rot and I didn't want to lose all the food, medicines and drugs, and even if the colonists didn't harvest them I have bots and slaves to work for me while we play poker :V.
That would be "labels on floor" mod.

Ok, it sounds like you just don't have their priorities in-line and they are simply doing other things while your plants go unharvested until they die.
When you look at the work tab, click manual priorities and take a look at the numbers. Lower numbers are higher priority, and jobs with the same number are prioritized left to right. So if cooking and growing are both set to 3 colonist will always do cooking before growing. You said you have 20 prisoners, wardening is a task pretty far on the left of the job screen so on default priorities that's what colonists are going to do before things like growing. I would hazard a guess you only have one or two growers and they are also assigned to wardening which is eating up all their time.
