Modern civilized people fight so namby-pamby because
(1) they fear punishment following the deed
and (2) You can almost always rely on the people surrounding you to intervene and reduce the carnage.

In effect, true fighting has become a taboo. And thank goodness for that!
But remove these restraints imposed by civilization (as in Rimworld's genre), and the fights become brutal.
People in our world are probably LESS careful often because they know that if they bash their face in then they can get it put back together.
You can replace a missing eye with an Archotech one, in the real world, with healing time of less than one day? Wow, i didn't know that!
'cause you totally can in Rimworld
Replace it? Probably not. But you CAN ensure that you won't get an infection and die.
