Is there a surgury or implant that can address this?

Yes, if you remove critical organs then 'Frail' is no longer a thing you'll have to address.

Ok joke answer aside; not directly, healer mech serum can remove it, that or apply bionic arms/legs to compensate for the -30%, but the condition itself; no.

ok, this is the 2nd time ive heard healer mech serum for curing things otherwise not possible, like dementia.

i thought that thing only gave medical exp.

There are three different mech serums. Neurotrainer mech serums give exp for whatever skill they are labeled for. Healer mech serums will instantly cure the worst condition of the colonist with no downside. Ressurection mech serums will revive a non-decomposed colonist, with a good chance of killing them again.

Ancient dangers on maps, or as a world quest reward, only options. It's rare in both cases.

I believe a bionic spine will solve your problem.

replace all major parts of your colonist would extend their lifespan, but they would still have some problems. For example, their immunity will lower as they get older so any diseases they catch will basically kill them.

Also you have to deal with dementia and other brain problems as they get older. its better to just use mods to help extend their lives one way or another.

Yeah, that dude's outta luck then. You could spend your resources cybering him up and he could still fall over dead of old age at any time. Just let him be frail and awful until nature takes it's course, is my suggestion. Try to find a younger replacement.

aww that sucks, hes my only guy with crafting and construction above 10, and his are actualy 17 and 18 lol.

he had a catacerac or what ever it was in both eyes, i put bionic eyes in him.
he had a heart attack, so i gave him a bionic heart.
now hes got the frail, bad back, and asthma in both lungs, oh and a bear ripped his leg off when he was trying to tame it, so he has a bionic leg now lol.

and he has dementia

There's no death by old age in RimWorld. Frail, Alzheimer's and Dementia are the only age conditions you can't cure/block with a bionic (or transplant), and none of them are fatal. Eventually age might slow immunity gain to the point where it's impossible to cure any diseases they get, but with a decent hospital and doctor I think reaching that stage is beyond what anyone has ever put into one game. Slap a bionic heart in a 100 year old and they will live just as long as any of your other colonists.

100 years old! Congratulations!

It is time to let go.
Build a nice, expensive cryptosleep chamber with gold pillars and patterned flooring and room for as many cryptosleep pods as you think you will need.

Send him to hibernation now, before dementia or illness and when you get his pod off the planet maybe there will be something to extend his life. Maybe at the Glitterworld planets.

Lucifer is also the final method to solve any old-age problems.

1 lucifer per 6 days, if you cant afford the upkeep them putting him in cryptosleep helps until you can afford to continue feeding his addiction.

Luciferium,,,

If the op is playing a modded game, the most popular cyber/bionics mods can basically make it so you stop aging all together in a roundabout way. Can replace everything.

If not Luci; then the easiest solution might just be to clone him; easier on colony wealth than cybering him up to the point it's a full robot....I saw a YT-playthrough about this recently and that colonist alone was worth way over 50K (something like 90.500 or so).
Alternatively...Rimworld of magic simply let's you "cure" dementia as long as you have a high enough leveled priest. That being said....it does NOT cure the deterioration of immunity though...but necromancy can :P
