How long then? Well, roll the dice, it's only a pawn's life I guess.
Nope. The moment a heart attack shows up on a pawn it's life threatening and they could die at any moment. The chance of them dying increases for the duration it's untreated until 100% severity where it's 100% fatal. You could get lucky for a while if you keep treating the heart attacks very quickly after they show up, but eventually your pawn will die before you can do anything about it.
The bad effects like chemical and brain damage or heart attacks is caused by tolerance - not addiction and tolerance goes down over time of not using the drug.
So you can use any drug once without worrying about health damage, but the chance of addiction is high except for smokeleaf / beer and wake up. You can use the social drugs every 2 days without chance of addiction. Taking any drugs every day would build up massive tolerance and cause health damage and you are likely to get addicted the second or third time you use it.
Addiction means you need a dose roughly every day or they get withdrawal effects which is not just a mood debuff.
TL;DR its very easy to get addicted, only the social drugs are really safe every 2 days. Tolerance is actually what kills or hurts your pawns and it goes down over days.
