#1. Mine. Sometimes passages are hidden behind mining spots.
#2. As steam poster Supergtp suggested, you may have to look up high, or even low for it.
Due to the random map seeds per game, the locations will be different in every game, so even if you "follow" a video on youtube, it might not be correct for your map seed in the location it is shown.
Yes, the layers or "floors" can overlap on the map. I had this happened to me before. It is one of the things that can happen in random map seeds.
Pickaxe the gate (takes a while), and then behind it was a normal dirt-filled passage. The Orc Camp was behind that.
I haven't done much at all upstairs yet, but I'm pretty sure that will hide the orc camp when I explore it.
