Annoyingly, you can use a mouse and gamepad simultaneously in the menus... just not during gameplay. Makes no sense.
I have a workaround though if anyone is interested.
I set the Left Trigger to Hold Aim Down Sight (though you have to change the keybind in the IW control settings to a letter or number rather than right mouse button - you cannot bind a mouse button to your controller - so first change the bind in the settings in game).
I also set the D-Pad to be E, 2, 3, 4... for optional controls in DS4.
Save the DS4 PROFILE.
Here is the stupid part: ANY control you set as a keybind on your controller - you HAVE to go into the game settings for controls - click the control that you want to bind to the controller buttons/sticks and hit the controller to activate its use.
So, for Left Toggle Stick Forward you have to go into Controls in IW and click the W forward keybind and then hit the stick forward to set it to W and in this example the bind wiill change from "W" to "W or W" lol...
You have to do it EACH TIME YOU LOAD THE GAME... and any tme you switch between modes. Once you've set it - you can go with it until you restart the game or switch between campaign, zombies, mp.. etc..

Its frustrating and insane that they never took the hybrid controller players into account when they ported this garbage.
The reason you have to enter it every single time is because it sees the controller input that you've adjusted in the menu as a duplicate. Since Up on the analog stick is "W" as far as the game is concerned, it sees two W's (you'll notice this when you change inputs; it says "W or W"), and as a result, it erases the second W, which is the one from the controller.
Solution to this is to bind the analog stick to keys you don't use via DS4. For instance, I used 7 8 9 and 0 for W A S and D, respectively. Then in the game menu, simply rebind using the controller. This will elimate the duplicate problem and the game won't erase your settings.
I tested this and found that even after exiting and relaunching the game, my bindings remained and I was able to play without having to do anything in the menus. I haven't tested this in each mode, but (fingers crossed) my suspicion is that it will work in general and be a permanent solution, so long as you have the correct profile active on DS4 as you launch.
Hopefully we'll see a patch in the near future that removes the need for such a stupid workaround.
UPDATE:
Solution works permanently across all modes, even if you switch between them or exit the game altogether. It works for MW: Remastered in both single player and multiplayer. Once you set your bindings, they are permanent and do not need to be done again.
