Double graphic settings BUG [SOLVED]
My menu under options -> video -> graphics for some reason show ALL the different graphic settings two times. It looks normalt at first since all the settings are shown in the correct order, but if I scroll halfway down, the exact same settings show up again in exactly the same order.

I can edit them independently - meaning if I for example change the first Vsync settings to "ON", the second will still stay "OFF".

Earlier I have been editing the user.settings file to fix some shadow issues, but I have now replaced it with the original one, but it doesn't help.
I also only have one mod installed (texture mod) which shouldn't interfere with the game menu.

I don't know what causes this, but if anyone has any idea how to fix this, I would appreciate that.

Delete both user.settings and user.settings.bak. Then restart the game and see if that helps.

I reinstalled the game and that fixed it. But I will try your method if it happens again. I'm gonna tweak the same settings the same way as before and see if that caused it.

So I found out what caused it.

The mod The Witcher 3 HD Reworked Project (link in the bottom) asks you to overwrite your bin folder in the Witcher 3 install folder in order for some 4K textures to work. When you overwrite the bin folder, it changes the graphics settings menu as described above.

After removing the new bin folder and verifying game cache integrity, Steam downloaded the original bin folder, and the problem disappeared.

I know you got it fixed by now but that screenshot of yours looked perfectly normal to me. Or am I missing something?

The thing you're missing is that the Vsync is the first setting and hardware cursor is the last one. After hardware cursor there shoulnd't be anything else. I took a screenshot of where the settings started to double so to speak. And yes, there is already a Vsync setting when I scroll to the top.

Hey, no worries at all. I know it's not easy to see from the screenshot.
Hopefully, the mod author will fix this. I have made a bug report about it on Nexus.

If you have The Witcher 3 HD Reworked Project mod installed delete "The Witcher 3\bin\config\r4game\user_config_matrix\pc\rendering (Backup).xml" and "The Witcher 3\bin\config\base\rendering (Backup).ini". This should fix it.

This helped me, thank you.
Just having an extra file
"The Witcher 3\bin\config\r4game\user_config_matrix\pc\rendering <whatever you name it>.xml"
caused the weird behavior for me. I had a backup file I manually created that was causing the issue. I simply renamed the backup: "rendering.xml.bak" and everything went back to normal.

I also have this problem except the files suggested to delete above do not exist.
I have a rendering.ini file in one location (The Witcher 3\bin\config\base), but deleting it has no effect.
I am running HDRP Next Gen version.
The game runs great it is just today i noticed the graphic options were duplicated which has not happened before.

This oddly just appeared to correct itself. a PC reboot and a couple of game starts and it simply went back to normal without me doing anything.
I was doing a lot of modding at the time so i may have done something to affect it but no idea what, but all is working great now!
