Resolution doesn't save post 4.03 update
Ran the recent update and now my resolution keeps reverting to 2560x1440 in-game and everything is mildly blurry. Even if I change resolution to 3840x2160, it does not apply and the textures remain blurry. I have mods but it's related to gameplay mechanics, not graphics so I don't think those would be the issue since it has had no effect since the next-gen implementation. Playing on DX12.

delete all ur dx12user.settings ini file from ur documents/the witcher 3/ folder and change in game folder /bin/x64_dx12/witcher3.exe to run with administrator privilage.

you will have to change all ur graphic settings again in game menu.

I noticed my mods folder was locked. So maybe make sure your Witcher 3 folder and all it's contents isn't locked ?

Like it actually has a lock icon on the folder? I just looked at mine and looks normal, no change to it.

What you on Windows 10/11 ?

Use steam's Browse Local files option to go to the Witcher 3 folder (Right click - Manage - Browse Local files I think it is)

Go to the parent folder Right click The Witcher 3 -> Properties.

Attributes Uncheck Read Only and if asks apply to all sub folders.

The only other thing I can think of is locate the settings where resolution is stored and check if the setting is actually saving.

For sure one of your mods is causing this problem, if disabling/deleting them not fix this issue then you should:
~ Mods or changes made to game files can cause various problems. The Steam/GOG verify/repair option may not recognize these mods, and even saves can be broken due to some mods used during the game progress. In this case, you may need to run an older save or start a new game. Uninstalling the game does not remove mods from game files, so you have to manually delete the entire game installation folder to ensure that no mods are left behind.
and then you can install all your mods one by one and you will find out which one is causing this issue.

The Uncheck Read Only keeps reapplying each time I open the Witcher 3 Properties folder.

I've disabled mods, loaded a previous save from weeks ago, and started a new game. Same issue.

Sorry but I'm not about to reinstall this game. I'm confident something else is broken. I just compared loading between DX11 and DX12 and my issues are isolated to DX12 launcher. Loading DX11, nice, crisp 4k graphics and details.

I'll likely have to continue in DX11 now since DX12 only loads under 4k resolution. So much for testing RT and DLSS in Beauclaire.

check what settings values u have inside:
\bin\config\platform\pc\rendering.ini and paste it here.
and also platformgameplay.ini.

if you deleted mods then in this pc/ folder you should have only platformgameplay.ini, rendering.ini and GalaxyPeer.json file. If you have more then there are mods settings.

But i agree, this setting should be also disabled at FSR, there is no point to use it there, what CDPR is thinking? I read here few threads about no difference between FSR and TAAU and ppl allways having dinamic res. scalling On...

Dynamic resolution scaling is set to "on" but it's grayed out. Had to disable frame generation to be able to turn it off, resolution still reverts back to 2560x1440

When is grayed out then it's disabled.
