I think it goes as far as Thaler beign alive no matter what, although it´s a possibility that he could died in Witcher 1 (a decision that is carried over to the sequel).
I personally think this details are pasable. I mean, CDPR are absolutely not the perfect devs some people tend them to be, but they still deliver a very complex and vastly game. In this case, some sacrifices or cut content were necessary.
That said, I also think that the overall quality of the main story is not equally pasable as these things, the ending beign particulary rushed.
