I couldn't find his horse outside as the work-around mentions yet I still tried the rest of the steps.
Apparently this has been broken since day 1. How on earth has it not been resolved yet?
There has been mention that it might be caused by people finding the cave before actually walking through the front entrance. I have to admit, I err, fell in from above. Heh. Anybody else? Can we find a common element here?
Another one might be - I only today (after 190+ hours in) ventured into the Nilfgardian war camp. I've been exploring so much and doing so many side missions that I still have quite a lot to cover.
Ideas?

I'm a level 22 or so and I had not visited this location before doing this question. And when I arrived, I arrived on horseback, and I was following the dotted white line which of course is via the road when on horseback.
So I discovered the marker and the cave at almost the same time, then dismounted and walked inside to find I could not talk to the character and the cutscene wouldn't play.
I also don't see Vernon's horse outside or anywhere else either. He is just permanently standing by the camp fire.

Err, no. The developers need to fix this or give us a way to reset it or I want my 208 hours of life back. :P
If the only solution is to start over there will be tens of thousands of people not finishing the game.
A large quantity of people (me included) have sent bug reports/tickets directly to them with the saved game files attached. It appears they have replied to every one. The responses are a bit generic but it's good that they are being acknowledged.
I was a bit disappointed that in their 1.07 patch notes it said that they had "Fixed a rare bug" when it clearly isn't that rare, but at least they are working on it and they have the relevant information.

Errr.. I don't remember him ever being there by the big fire.. When I went to see him, he is down the far left end as you enter the cave.

I'm sorry for the time you had to wait for a reply and thank you for your feedback. We are currently investigating this issue.
There's a possible workaround for this issue, please reload a game save in which:
1. Geralt doesn't know yet where Vernon Roche is and Dijkstra is about to tell him his location
2. Temerian Partisan Hideout hasn't been yet discovered
3. Geralt is in Novigrad
This workaround worked for some players and it's worth to give it a try.
