I also had a mildy amusing bug in the quest as well. After saving her and finding a surviving officer on the ground, Geralt, Roche and the girl say he is the last survivor and you get the choice to either kill him or not. And then I see the fella in the back looking right at the camera and not moving. Geralt and friends don't seem to realise they missed the guy in the back bahahah

yes, and i think it's even needed to complete some main story quest later.. so i'm quite worried. I'm already back to Kaer Morhen.
"If he’s not there, and you have the quest A Deadly Plot active, go meet with Dijkstra at Passiflora to get Roche to go back to the waypoint. You do not need to complete A Deadly Plot to return Roche to his position – just have the initial meeting."
