Petri's Philter forumla location

Anyone know where to find the standard Petri's Philter potion formula (sign intensity +)? I have the 2 upgrade version but not the basic one i can't find it anywhere.

I found it at the attic of one of the houses in Novigrad. I know that doesn't really help since that city is gargantuan. Sorry. :|

It must be randomized, unfortunately. Here I am trying a sign build, and I can't find the single potion formula I need most. -.-
