You can dismantle it from monster loot, cant quite recall which one but if your a looter like me your bound to find one in the dismantle screen.

i found 1 in a ghoul but it was like 1 percent drop rate must be another way to get more

For some reason I found that the creatures in crookback bog dropped a lot of eyes. Could be random though..

Just dismantle them from named monster eyes,

its not rare. any monster eye dismantle it
