Why people suggest to have V-sync off?
I tried playing the game with on and off. When it's off the image is really bad and i think i get many fps drops. (Like 10-15 frames up and down).

just play on the settings that makes the game play smooth. many people have diff. pcs and that what changes everything.

You will get some tearing but vsync lets the game run as fast as it wants or can. It can do 55 FPS or 65 FPS. With vsync on the game will always run at a multiple of the refresh rate. Can't hold 60 then it goes to the nearest multiple of 60. This can cause stutters b/c your eye is seeing 60 at one point and 30 at another. Vsync is the enemy of all gamers and performance minded enthusiasts. Its just wasted frames while waiting for the blanking signal so the screen doesn't redraw during a screen refresh.

Like what other's have said it's different for different pc's. I've had some GPU's that most games looked and performed better with vsync on and others that most of my games ran better with vsync off. It's something I always check every time I upgrade my GPU.

Because if your GPU can't keep a solid 60+ on a 60 hz display, then it will drop to 30fps, then back up to 60, then back down to 30, over and over, and this fluctuation can be far more distracting to people that are sensitive to it, then the tear is.

Tearing will be visible on scenes with long vertical lines such as looking at columns, poles, houses, etc. Tearing isn't as noticeable in the horizontal plane so horizon scenes and outdoor running (not in a forest) isn't affected too much.

Cut scenes can suffer b/c the FOV is usually set to 20 or 30 which is very close and makes the characters gigantic.

I understand what you are saying but i just experienced the exact opposite thing. As i state above with v sync on i usually play @59-60 fps. I only get some drops in big cities or rarely in fights. But not big drops. Like from 60 to 54 and then back again.
When i turned v-sync off and i uncapped fps i 've been getting up to 70 or something fps but with huge drops (50-70) which is pretty annoying.

In general v-sync with cap your fps to the refresh rate of your monitor. This can help reduce screen tearing and other artifacts when you are getting a fps higher than your monitors refresh rate,, On the down side if you are only pushing fps way under the refresh rate this can also cause artifacts and problems..... I would try with it off first and see it the game plays smooth, with no tearing or whatnot...

I personally found that running with in game v-sync on did not solve my screen tearing issues so i turned it off in game, and enabled adaptive v-sync through the nvida control panel and that solved my issues, in this case adaptive v-sync will enable v-sync when you are at or above the monitors refresh rate, but turns it off if and when the fps drops below. So now i have a smooth gameplay experience as my fps sits between 50-60 (60 being my monitors refresh rate and when v-sync kicks in)

Some people have monitors that have refresh rates much higher at 120 or 144 hz so in that case they would suggest having v-sync off, because they would not be hitting that in terms of fps so for them v-sync will cause issues.

It depends on what your monitors refresh rate is as to what extant you need v-sync and to what extent it might help or cause issues...

I have a 60 hz display, so I don't run into that problem. Perhaps if I had something like a 144 hz display I would have a different opinion(no clue, as I have never had one).

I have a 60 hz display, so I don't run into that problem. Perhaps if I had something like a 144 hz display I would have a different opinion.

I have a 60hz monitor myself. I don't know why but i find it really annoying to have such drops even when i know my monitor doesn't support more fps than those.

Question. I cap my fps to 60. When i uncap them to unlimited i get terrible drops as well. Is this normal?

With vsync off or vsync on you can get frame drops. Vsync off results in a more variable frame rate over time but the difference between FPS might be more minimal. So if you vary between 60 and 50 with vsync off and 60 and 30 with vsync on the former will result in a smoother experience.

Vsync on / off isn't a magic bullet. Adaptive vsync is a cool invention that gives you the best and worst of both worlds but the bottom line is you can't squeeze apple juice out of a pear. In other words if you physically cannot run the game at a constant 60 FPS no amount of magic will make up for the hardware. Very few games maintain 60 FPS at all times. Smooth gameplay is about smooth and consistent FPS over time, not high FPS in short bursts. Max FPS means nothing.
