AFAIK finishers are automatic in vanilla, but you can increase their probability with certain elements like tropheys or skills that literally tell that they increase the chance to kill an enemy with a hit.
Note: you must WALK up to then. Running up to them will result in a light attack instead of a finisher. You will see the prompt then the enemy e.g. human or harpy is knocked down.
That's Coup de grâce actually, but yes that is how that is done. I believe the OP means the occasional dismember, decapitation and the like. I believe what animation is played is random/position aware but to get them my understanding is critical hit/fatal hit. Please correct me if I'm mistaken.
