Hitboxes are technology responsible for counting attack as successful in melee combat. E.g. when you hit an enemy with a weapon hitbox responsible for detection of successful hit of enemy's body(3d model). When you have bad hitboxes in the game attacks can be counted even when attack doesn't touched the body.
So damage or even death happens not because of successful attack, but because of a bug. Here is another video example:

Reason why you don't have this problem when making damage is because Geralt has descent hitboxes. While many enemies have horrible one.
But W3 has very terrible hitboxes, which is a glaring issue. Yet after all those years they still haven't even tried to fix that. That's not a walking sim either. Witcher 3 is action-RPG and combat is the core gameplay of the game.
If only mod makers figured out how to edit hitboxes in W3.
