-Journeyman armorers can upgrade your Witcher school armor to Superior level, but you need to go through Enhanced level first. Past Superior level, you need a Master Armorer.
-The actual upgrades aren't unlocked until you have the diagrams for them, and those are unlocked through the Scavenger Hunt treasure hunts; merchants will sell the maps.
-Once you have the diagrams, all you need to do is to visit an Armorer. If the recipe's available, you'll see what raw materials you need.
Btw, the Ursine armor isn't always "the best". Each of the different school armors is really good for a specific playstyle, and the playstyle varies between schools. Cat School is primarily tanking, Griffin School focuses on Signs, and Ursine School is all about tanking. So if tanking is your playstyle, then Ursine's what you want. But if you favor a different playstyle, go for the school armor that suits that playstyle.
