About the dragon.The dragon is very exceptional monster that is not allowed to be killed.
It's the code of the witcher.All of them were taught to don't bother with any dragon.
So they don't normally kill dragons, godlings, trolls, etc.
Why?
That's not to say that trolls could not be dangerous (just "deceipher" what named troll has to say), but they are not agressive by nature. They only use an logic that not always work out that great for humans...
So - no, an Witcher does not kill every monster (human or otherwise).

When i play geralt, i always choose the most difficult path and try not to kill neutral/good monster or convert an evil monster to a good one.
Lambert kill eveything for exemple .
I think it's more a geralt thing and his personnal code. He will never kill a sentient being if he can avoid it, he won't go after dragon because only a few of them are left and they have been heavily persecuted in the past.
Geralt is a neutral kind of guy and his world is not black and white but a world of grey.
That's probably why Geralt is one of the character i like the most in video games/books, his life is hard, like crazy hard and he still never choose the easy way out.
