Hmm... Maybe if you don't go to help her in the second game instead help Saskia (or the other option witch I don't remeber)... Maybe it is something from the books...I've not read them.
Triss probably has zero actual scars because she can heal her body at any time. Again, this is why she doesn't age, despite actually being over 100 years old. But as has been stated before, she still sees those scars in her mind, in much the same way a girl who was once ugly may not fully appreciate how beautiful she's become later on in life. It's all in her mind.
Lambert asks "How are the scars she got at Sodden?" It turns out Triss suffered a major injury during that battle and had some pretty big scars on her chest/breasts. This is why (in the books at least) she does not wear any clothing with even a little bit of cleavage. But as TSYR119-A, pointed out, she might have concealed/healed them, but i am not sure. If Geralt would have answered that question, it would mean that he saw Triss naked, or at least topless, and that in front of Yennefer. Eskel immediately pointed out that it was not a funny question.

Yennefer was also severely injured and became blind. She had to heal her eyes, a very long and painful process.

So it maybe no scars, but mental trauma after she saw her mutilated body after Sodden and before healing. And she done with this trauma in the end of "Lady of the Lake".
