Two big zones and two smaller ones, like ydraig said.
And when I say big....I mean BIG ASS ZONES.

another zone with a group of islands (you have to travel by ship to get there)
these two are really big and you spend most of the time there.
Then there is a prologe-zone with a village but you only visit it in the begining. you can go back there when ever you want, but it doesn´t make much sense.
another zone is the emperors palace, used for 2 or 3 scenes. you also can visit it whenever you want.
Last zone is the witchers homecastle with it´s suroundings. Some quests take place there and it has to be a different zone, as it is far away from the country the mainstory happens.
So in total there are: 2 LARGE zones, 1 medium-sized zone (tutorial land), 2 small zones (Kaer Morhen and a certain "isle") and one TINY zone, the palace in vizima.
It definitely is an open world game. It's also very much a story-driven game and, unlike TES games for example, is not really a sandbox game.
