'Reason of State' quest failed at start of 'On Thin Ice' quest?
wtf? I completed this mission already but failed it when I started the on thin ice quest. :( Any ideas?

Hmm... Did you started your game with 1.07 patch?
Did you use any mods or console commands?
This game is on PC, right?

It happened to me as well but the story ends correctly as a result of your actions for that quest. Also noticed the Triss and the Statues mini-quests hangs without completing even though its completed in visuals. Small bugs but noticed nothing bad from them.

Yeah I got those quests but after speaking to Triss about them they don't trigger? :/ Bugged.

Given the bugs with achievements and the ongoing bugs with some quests not triggering like these quests here, I bet the next patch will address them. It's not a game breaking bug from my research, as the events unfold correctly, and after completing the game the story does end correctly depending on how you finished the quest. It just remains greyed out in your quest list.
