Using mods to bypass the game mechanics is not a fix.
Not an fix, but it is an possible solution to get out of the stalled situation. Thus the poster is right here. I still have to check myself if the crystal is missing so no need to do it yet.

I was having the same issue but I solved it. I watched IGN's video on all the Wolf Gear locations because I wanted to see where the portal took the witcher. In the video it shows the witcher jumping through portal after getting the 2nd crystal and activating it. He falls into a cave and fights a wraith then picks up the diagrams. Then the witcher walks to the mouth of the cave and you can see clearly Kaer Morhen across the way. In game I looked from the Kaer Morhen side and saw the mouth of the cave about half way down the mountain directly under the half activated portal glowing above. I actually climbed up from the ground and made it into the cave and retrieved all the diagrams with the portal above still only half activated. I am using patch 1.10 also. It took a while to do this but I did it. Pretty cool!

Alright, so I took a different approach to this. I already have the debug console downloaded (which you can get here http://www.nexusmods.com/witcher3/mods/143/?), so what I did was give myself godmode and slid down the mountain from where the portal is at. It was really a matter of luck that I got to the cave, but I landed in it and got the diagrams. As far as I'm concerned, this problem is over for me, lol. Now I just have to find a way to fix my Berengar's Blade problem.
