Just done the main gwent quest won all 4 matches didn't get any cards and yes i did look through the 4 different rhealms.

The High Stakes quest gives you leader cards only (4 in total, 1 for each deck). You sure you checked if you got any new ones there?

The Steel-Forged - Northern Realms
The Relentless - Nilfgaard
Queen of Dol Blathanna - Scoia'tael
Destroyer of Worlds - Monsters

I have been trying to get the achivement have played the game 4 times still cant find all the cards.

There is a merchant you have to free from a bandit camp in Velen that sells a couple of cards you may have missed

Yup, he is the amateur armorer in Mulbryvale, held captive just southwest of there, on the riverbank. Well he has one anyway...

Oh, and afterward it is my favorite place, Mulbryvale, while on the continent, to pop in and get my gear repaired and enhance weps and armor, if needed.

It is very quick, you come out of fast travel headed in the correct direction, just a fast sprint and a quick right and you are there.

also, too, you have to buy all the cards available from innkeepers who do sell them; the part most miss is the one in White Orchard... you have to visit her 2 additional times after buying out her first stock, she gets 2 more decoys. If you finish the main quest in White Orchard, you miss the chance and cannot get them afterward.

You can get the cards she sells even after she's gone. The merchant in White Orchard, next to the river (the one you save from the griffin in a cutscene at the beginning of the game) sells the cards once she's not there anymore.

"Base Version"? not sure what you mean. Do you mean the base deck you first get and must use to beat the scholor in White Orchard?

He's talking about the achievement which says something like "collect all gwent cards in the base version of the game".

"Base version" in this context means the original Witcher 3 game...which strongly hints at the fact that the expansions will add new cards and/or decks (and have seperate, new achievements).
