So I found 3 separate posts on 3 different threads where people specify that they got theirs in White Orchard... which is kind of odd. I even found a video playthrough where a guy gets it in the bandit camp SE of woesong bridge in White Orchard.
I wish I knew how the spawn system worked in this game for loot.
For example, now I returned to white orchard at level 36, and I found a level 33 sword in a chest. There is no way that sword was level 33 when I left white orchard. So, what gives? Does it spawn initially with the world? Does it reset when you level?
But once I've found a level 34 sword when I was level 16, from a chest guarded by a ?? monster that I snuck past to grab the loot. So I dunno why that happened and it's the only time I've seen something like that with the loot.
This loot system reminds me of the loot system in Final Fantasy 12 (Or whichever was a ripp off of Star Wars and had a character named Vaan). A lot of people complained about it because it was better to wait towards the end of the game to open chests.
I found a poster on reddit that said that once you enter the world, the items are set, but their levels may change. For example if you save, check a chest in white orchard that has a level 3 sword, cheat engine yourself to level 36, and the same sword in that chest will have leveled up.
I'm not sure what you mean by that chest giving you enhanced cat. Are you saying that it does so, on the same save? If so that isn't surprising or new information. The chest I mentioned where in the video he got the Ancient Leshen decoction, it's the bandit camp to the SE of the Woesong bridge. I just restarted a game to check this chest, and it had the chort decoction for me.
I think the way the chest loot system works is something akin to oblivion, where "boss" chests have the best loot, but there were various kinds of boss chests, i.e. vampire dungeon boss chests, monster dungeon boss chests, etc. Each of those had different tables.
edit: this is to say that so chests may be guaranteed to give you some manuscript page, some chests may be guaranteed to give you a weapon, or an indredient, etc.
Maybe not for recipes but I have always found swords in the chests suggested when looking online. Tir ToChair can always been found in specific chests (not 100% likely to show but it does appear if you save/reload enough). Maybe recipes are set up differently so not to have repeats. So say if I have superior cat already and look in a chest it will just give me something else instead.
