Any other games like this ?
Just love the question marks and the contracts ....put over seven hundred hours into this gorgeous game ..still loving it on the tenth anniversary play through, after not touching it for half a decade. Doing the w3ee redux this time ...medieval fantasy third person exploration is what I'm after after.

I just started it, so I don't know for sure about exploration yet, but I'm playing KCD2 which has a lot to explore - I just don't know yet how well they filled it. It's first-person but there's a mod on Nexus for third-person - I watched a video of it and the animations looked good, but I haven't tried it yet.
Kingdom Come - Deliverance is first person, but it is the closest I have seen to The Witcher (down to several game mechanicts, like intelligent horses, the stash, saddlebags). The world is also beautiful, best woods in a videogame.

ac valhalla, though it is 1-2 levels below witcher, it's a newer game and kinda compensates

gothic 1 and 2 its better than withcher 3
AC Odyssey is the closest AC game to Witcher 3. Why? Because a lot of the side quests and decision mechanics in both game were done by a guy named Nikolas Kolm. He wrote most of the Triss romance quest as well as the quest where Geralt makes the bad decision to try to romance both Triss and Yen and gets chained to a bed. In Odyssey he wrote the Kara set of quests which is one of the major side quests that feels like it's part of the main story. and several other side quests and the decision tree that decides which ending you'll get,

Interesting interview with him discussing both games. The meat of the interviews starts at 23:13

Short answer, as far as I am aware TW3 is just too good for there to be much competition, especially when you require it to be in the medieval setting, I think the first thing that came to mind is Skyrim. That being said, I do not think the quality is close to comparable at this point, the Witcher 3 has sort of ruined most other RPGs for me because they simply don't feel as good. Another game you might want to check out is (as already mentioned by someone else) Kingdom Come Deliverance, this game is simply great if you are someone who can adapt to the mechanics of that game, it's highly realistic and the combat is definitely not intuitive and you will struggle, a lot, especially at the start. Either you will hate it or love it is my guess.

If you are willing to explore outside of the medieval era into a somewhat more early industrial period(?) setting, you will probably have the most fun in RDR2. That game is imo the biggest rival to sheer quality, open world exploration, story line, and just general experience of Witcher 3. I think that this will give you the same sort of experience as the Witcher3 might have done, but just in a cowboy setting. If you haven't tried it yet, I highly recommend you do it, its one of the best RPGs ive ever played together with Witcher3.

Maybe try Elex 2

Dragon's Dogma 2

The story and NPC interactions are lacking, but I had a great time with the combat and vocations (classes). I was surprised at how much I enjoyed the game despite the story / character cons.

Gonna sound strange, but Ghost of Tsushima. Game has quite a few similarities to Witcher 3, like its stylized game world, quest structure, combat (though more technical in Ghost, of course), and there's even a lot of instances where you track people and animals using heightened senses. Maybe it's just me, but that game has similar vibes and mechanics to W3. They're also similar in their very bleak and gritty stories and characters.

Oh, and Ghost also has armor sets you unlock that you upgrade over the course of the game, each coming with its own set bonuses (like Witcher gear). I know it isn't European medieval or fantasy, but that's my suggestion.
