If you are willing to explore outside of the medieval era into a somewhat more early industrial period(?) setting, you will probably have the most fun in RDR2. That game is imo the biggest rival to sheer quality, open world exploration, story line, and just general experience of Witcher 3. I think that this will give you the same sort of experience as the Witcher3 might have done, but just in a cowboy setting. If you haven't tried it yet, I highly recommend you do it, its one of the best RPGs ive ever played together with Witcher3.

Dragon's Dogma 2
The story and NPC interactions are lacking, but I had a great time with the combat and vocations (classes). I was surprised at how much I enjoyed the game despite the story / character cons.
Oh, and Ghost also has armor sets you unlock that you upgrade over the course of the game, each coming with its own set bonuses (like Witcher gear). I know it isn't European medieval or fantasy, but that's my suggestion.
