Spoilers galore. Read at your own risk.

Here are the different ending states you can get, depending on the decisions you make:

1. Ciri:
-- If you drink with Ciri, accept Emhyr’s coin, go with her into the Lodge, give her Lara’s necklace and stop her from visiting Skjall’s grave, Ciri disappears in the end.
-- If you have a snowball fight with Ciri, refuse Emhyr’s money, let her go to the Lodge alone, allow her to destroy Avallac’h’s lab, let her visit Skjall’s grave, take her to see Emhyr and side with Thaler and Roche, Ciri will become empress.
-- If you have a snowball fight with Ciri, refuse Emhyr’s money, let her go to the Lodge alone, allow her to destroy Avallac’h’s lab, let her visit Skjall’s grave and not take her to see Emhyr, Ciri will go her own way, living as a Witcher

NOTE: See end of post for more information on Ciri.

2. Geralt
-- If you complete the Now Or Never quest, tell Triss you love her, choose to be with her and ignore Yennefer, Geralt ends up with Triss.
-- If you complete The Last Wish quest, choose to be with Yennefer and ignore Triss, Geralt ends up with Yennefer.
-- If you complete both the Now Or Never and The Last Wish quests (or ignore them both), and tell Yennefer and Triss you want to be with them (or ignore them both), Geralt will end up on the road, by himself.

3. Skellige
-- If you complete the Clan An Craite quest and choose Cerys in the King’s Gambit quest, she gets to rule Skellige, which leads to prosperity.
-- If you complete the Clan An Craite quest and choose Hjalmar in the King’s Gambit quest, he gets to rule Skellige, which ends in more bloodshed.
-- If you ignore the King’s Gambit quest, Svaringe becomes king and fights back the Nilfgaardian forces, uniting the clans and creates a powerful dynasty for years to come..

4. Novigrad
-- If you ignore the Reason of State quest and the Assassin’s quests, Radovid will take Novigrad.
-- If you complete the Reason of State quest and the Assassin’s quests, then side with Thaler and Roche, Emhyr van Emries will become ruler of Novigrad.
-- If you complete the Reason of State quest and the Assassin’s quests, then choose Djikstra, he will take over the city.

5. Emhyr var Emreis
-- If Emhyr takes over Novigrad, he gets to live.
-- If either Djikstra or Radovid take over Novigrad, he dies.

6. Sara The Godling
-- If you make a deal with Sara during the Novigrad Dreaming quest, she will stay in the house with Corrine.
-- If you throw Sara out during the Novigrad Dreaming quest, she will meet Johnny and stay with him.

7. The Mages
-- If you complete the Now Or Never quest, Radovid will stop hunting mages and turn to nonhumans instead.
-- If you ignore the Now Or Never quest, Radovid will keep on trying to root out magic users.

8. Keira Metz
-- If you convince her to go to Kaer Mohren in For The Advancement Of Learning quest, she’ll end up adventuring with Lambert.
-- If you let her talk to Radovid during For The Advancement Of Learning quest, she is executed. This will create a new side quest later in the game which Triss will give you, to accompany her to Novigrad to retrieve Keira's body.

NOTE: See below for more information on Keia.

9. The Bloody Baron
-- If you kill the tree spirit in The Whispering Hillock quest, the Baron will go to the Blue Mountains with his wife.
-- If you set the tree spirit in The Whispering Hillock quest free, the Baron will have hanged himself.

--- Freeing the spirit:
-- The spirit will require three items: a horse, feathers and the spirits bones. Once all three items are collected Geralt will have to pierce the spirits heart thus releasing it and taking control of the horse. Unknown to the witcher once the ritual is completed the possessed horse will attack the people of Downwarren killing the villagers in revenge and then taking the children. Doing this effects the quest Family Matters, as when the witcher returns to the Witches of Crookback Bog he notices the the women known as "Gran" is really the Baron's Wife, Anna. If the spirit takes the children Anna is blamed by the Witches for letting them escape thus needing to be punished. In another quest related to Crookback Bog. Anna will be cursed by the Witches as punishment and die, thus making the Baron hang himself.

--- Killing the spirit:
-- Choosing to kill the spirit will lead with the spirit shielding itself with tree branches, cut those away to be able to attack the heart however the tree also summons spiders to attack the witcher. If the spirit is killed the people of Downwarren will not be harmed later however, it is hinted that the Witches of Crookback bog kill and eat the Orphans of Crookback Bog and Anna will not die thus the baron not committing suicide.

10. W h o r e s o n Junior
-- If you kill Junior during the Get Junior quest, the doppler will be successful in immitating him and improving his public image.
-- If you spare Junior in the Get Junior quest, he will end up in The Bits, miserable and hated.

NOTES ON CIRI:

Major Choices – Fate of the Swallow

There are three ways Ciri’s story can turn out in The Witcher 3: Wild Hunt. One of them is awesome, one is OK and the third is something we’re pretty sure you’ll want to avoid. Well, unless you hate Ciri, in which case this might be your preferred route.

Determining Ciri’s fate depends on five key choices. Each of these results in either a positive or negative reaction from Ciri. If you get more positives than negatives, you end up with one of the better conclusions.

Note: During the main quest, Blood on the Battlefield, Ciri will ask Geralt to go with her to fight Imlerith. If you agree to go you will not be able to unlock the Ciri is Empress ending, and you will not see the second choice. You can, however, still end up with a positive outcome.

-- While completing the Blood on the Battlefield main quest, Ciri will ask Geralt how to manage her negative mood. You can choose to have a snowball fight with her (+1), or you can tell her that she doesn’t have to be good at everything (-1).
-- During the Father’s Day main quest, the Emperor will attempt to pay Geralt for bringing Ciri to him as they agreed. Geralt can refuse payment (+1), or he can accept it (-1). Refusing payment results in Ciri being proud of Geralt.
-- While completing the Final Preparations main quest, Geralt can allow Ciri to speak with the Sorceresses on her own (+1), or he can go with her and speak on her behalf (-1). The latter will cause Ciri to lose confidence in herself.
-- The Child of the Elder Blood main quest will see Ciri angry and determined to ransack the laboratory of Avallac’h. You can allow her to do this (+1) or you can tell her to calm down (-1). The former will help strengthen her resolve.
-- Also during Child of the Elder Blood, Ciri will tell Geralt she wants to travel to the grave of a boy who saved her. You can agree to go with her (+1), or tell her that there is no time (-1). The second option will cause Ciri’s spirit to weaken.

1- If you have more negatives than positives, Ciri will defeat the White Frost at the end but will not return and is presumed dead, especially by Geralt. You'll then have the:

-- Something Ends, Something Begins I --
This ending doesn’t necessarily mean Ciri is dead, but it’s the worst of the three options if you want to see her healthy and happy. Geralt returns to Crookback Bog to fight the last remaining Crone and retrieve Ciri's medallion. After finding the medallion, he sits alone in a hut while it is swarmed and overwhelmed by hundreds of monsters. He does not react nor move to defend himself and is presumed to have just allowed himself to die.

2- If you have more positives than negatives and took Ciri to see Emhyr, she defeates the WShite Frost, returns alive and:

-- Something Ends, Something Begins II --
This ending is considered neutral by many and it can only happen if Emhyr is alive. If you chose to ignore the assassination plot to kill Radovid and he remains alive, he will defeat Emhyr. Once dead, Ciri can no longer become empress.

NOTE: If Radovid is dead, Dijkstra will try to take over. If he succeeds, he will defeat Emhyr and the same conditions apply, but for that to happen you need to walk away from Roche, Ves and Thaler and leave them to die at Dijkstra's hands.

3- If you have more positives than negatives and did not take Ciri to visit Emhyr, Ciri will defeat the White Frost, returns alive and become a witcher like Geralt:

-- Something Ends, Something Begins III --
More positives than negatives and Ciri must NOT have gone to see Emhyr. Regardless of Emhry's outcome, if she never goes to see him, she will stay with Geralt, train with him and become a witcher like him.

NOTES ON KEIRA:

-- If Keira is alive after For the Advancement of Learning, you learn she plans to speak with Radovid and try to bargain for her life. If you let her do as she pleases, Radovid will kill her.
-- If you talk her out of it and convince her to go to Kaer Morhen, she will become one of your allies there during the Battle. If she it present at the Battle, she will save Lambert's life and they will both head off together afterwards to search for a cure for the Catriona plague.
-- If you confront her about using you, it will lead to a fight in which you are forced to kill her. If she is dead, during the Battle at Kaer Morhen, Lambert will be overwhelmed by the Wild Hunt and you have to act quickly to save his life. If you don't, he will be killed.

And finally, Letho of Gulet, from TW2: Assassins of Kings can appear in TW3, but only if your TW2 imported save had him still alive at the end. To ensure that he does appear, you can choose to Simulate a TW2 save and answer some questions during your shave before meeting with Emhyr in Vizima at the start of a new game.

Letho will then appear during The Fall of House Reardon contract and lead to a small quest of his own, Ghosts of the Past. Once completed, you may recruit Letho as an ally by telling him to go to Kaer Morhen.
Is it all those quests or? cuz this is what im really after: "But to the question when will my decisions mather that much that they might kill Ciri? what quests will decide this?"

What will determine if Ciri "dies" or not is the following:

Blood on the Battlefield, you if you chose to have a drink with Ciri instead of the snowball fight, she loses confidence in herself. And if you took her to see Emhyr, if you accepted the coin, she will be angry and lose respect for you.

In Final Preparations, if you go with her to talk with the Lodge, she again loses confidence in herself, and believes that you have no faith in her to stand up for herself.

In Battle Prepartions, when visiting Avallac'h's lab, if you tell Ciri to calm down when she's angry at the assistant and want to strike out, that pent up frustration won't help gain any self-control later on. There is also an opportunity to give her Lara Doren's necklace which lessens her ability to stand on her own and step out of her ancestor's shadow.

And again in Battle Preparations, when Ciri asks you to accompany her to visit Skjall's grave, if you refuse and tell her there isn't time, she'll get furious. This is a very sore point for her. She is deeply emotional about Skjall and needed the closure.

All of these decisions serve to shake her confidence in herself. She has enormous power and is not able to control it. Geralt is key to supporting her but also allowing her to make her own decisions becaues he has complete faith in her. If that faith is shaken, she will still save the world but it will cost her her life.

Does that answer your question?

Blood on the Battlefield, Final Preparations and Battle Preparations are the key quests.
This is a rather old thread now but i check in on this whenever the quests comes around but at the part with Battle Preparations (spoilers ahead) i and ciri made a small mess at avallach's place but i went to the ship after the quest and i talked with ciri, yen, triss and Avallach and told him we went by his lab and all that, does this affect anything? he said he hoped we didn't make any mess (opss) but this is just "small talk" and wont affect anything right?
