Hearts of Stone gives you quests that are very clearly marked HEARTS OF STONE, and their map markers are even blue to differentiate from the normal ones (yellow). They're at a higher level than the rest of the game. What CAN happen is that you meet some enemies from the DLC if you go around Velen discovering locations (the "?" on the map). If you see spiders or some knights in red armor around level 32, they're the DLC enemies.
Blood and Wine takes place on a completely new region, and you need to progress through the story to even unlock that. So you will never stumble upon it by accident.
i guess CDprojeckt owns GOG so they dont know how to utilize steam's seller features properly.
but there's a different bundle called 'expansion' which is just the two big DLC's.
in my country it's actually cheaper to get the base game and the two expansions separately for some reason.
