And another question: does he immediately replace the Roach or do we need to do something?

I think OP is referring to when Geralt meets Emyre after the isle of mists, there are a series of choices that can replace 'Roach' into a black Nilfgaardian stallion.

I've inadvertently done it. I believe you'll need to take the path of 'not accepting the money' ..

I changed the default Roach to the demonic one by putting on a saddle,

and then to show you the Nilfgaardian one, I loaded a different save on which I had it (from doing the quest and choosing the right option):

So basically, on a single game save, you can have and change between the default and demonic Roach, or the Nilfgaardian and the demonic Roach. You can't have the default and the Nilfgaardian Roach on a single playthrough/game save. You have the default Roach by default, you get (change to) the Nilfgaardian Roach from the main story quest with Ciri, and you get the saddle that changes either the default or the Nilfgaardian Roach to the demonic one (when you put it on) during the final quest of the Hearts of Stone DLC.
