I was in a village, and discovered i couldn't cast sign's, i'd just finished a fight, and signs were working.
Since my signs worked b4 I got to the village, i fast travelled away from the village, and tried my signs again.....
They worked just fine, so I go back to the village, try and cast my signs, no go, fast travel away, signs work, I never did find out what the issue was, I think it was "The Byways"
Sounds like they haven't fixed the issue in any update.
There's instances where u haven't been able to mediate, even when the fight music isint playing, It just keeps saying "You cant do that now"
Unrelated, but similar enough, occasionally when swimming the "dive" button stops working, the only fix I found is to save the game, close and restart.
But ir's most likely a config/text file, these can be edited with notepad... I'm also guessing it's buried in the witcher 3 library some-where under "Meshes" or something. O did drag this up https://github.com/aschleg/witcher-3-enemy-scaling-and-other-changes/blob/master/mod/gameplay/abilities/geralt_stats.xml
Since you have modded the game, it's highly likely that one of the mods caused the problem.
Disabling mods does not necessarily fix a bugged save.
Try removing any and all traces of mods (you can simply move the folders and files out of the game directory, e.g. to your desktop), verifying the integrity of the game cache (or, ideally but possibly very time-consumingly, going the nuclear route by completely uninstalling and reinstalling the game), and then starting a new save from scratch.
If that new, 100% vanilla save does not have the problem, then it's pretty much certain some mod probably irreversibly damaged your current save.
