1 delete everything and make sure you only have original game nothing else , delete vortex dont need it.
(by delete I mean like 100% uninstall and manually delete files and in documents as well to ensure you dont have that one random mod that causes issues later on.
(uninstalling only is useless since all mods stay unless you manually delete them.)
now you can start by checking if the mod is for correct version of game (next gen or old gen , and whether it is for 4.04 since any older version wont work unless it says otherwise in description)
next you follow instruction that says in description and download the mod , copy paste to new folder , unpack the file , delete the zip , and copy paste to mod file of witcher 3 , and put dlc file in dlc folder.
for most mods that would be it , but some require extra steps that say in description or readme file that is attached to file you download.
now the important thing is that you cant just spam 100 mods and think everything works ...
some mods are not working with the other mods meaning you can only select one you want and not both , its important you chose only mods you need not spamming random.
next once you are sure you installed everything correctly you go to script merger . run the merger , make sure everything works there , and then exit , and run game , then in game it is recommended to start new game , but loading saves should also be fine.
if you have the mods that needed extra steps there would be a mod menu in options that you need to check and ensure mods are enabled there and that all settings are set correctly.
right now I would say you can only find max 20 mods on next gen that you really need but as time goes on more mods would be added to nexus.
