try to delete all mods manually run script merger , verify files on steam , and try to load a save to see if the issue was some mod.
or if the issue is corrupt save you can use some save on nexus there is a lot for new game plus.
(I have a save near the end game , it is not perfect or anything but it should work.)

run game from game folder witcher3.exe. sometimes an error msg appears not hidden by steam.
check windows event viewer and also windows reliability history. might be a game crash event or maybe system related errors.
been running on a 3060ti since the december update. started a new toon and now have a level 40+ toon somewhere at the start of DLC Blood and Wine. been playing on dx11. I dont see any difference using dx12. just using the settings the geforce experience recommended
Some things to try:
Verify files
Turn off V-Sync
Change from fullscreen to windowed borderless or windowed, or vice versa
Update/downgrade GPU drivers
I did these things (except updating/downgrading gpu drivers) and rarely get crashes now. Before, every load screen was a 50/50 chance of CTD and often locked my rig to the point of requiring hard restart.
Crash every 30-45 minutes.
