Is there any visual differences between DX11 and DX12?
Disregarding RTX of course. I'm about to install Witcher 3 again but I heard its running pretty poorly on DX12. I have an RX6600 and a Ryzen 5 5600 btw, I remember testing it out when the update released and it was running pretty smoothly with everything maxed out (minus RTX), but I heard the latest patch ruined the performance on DX12.

On the other hand, I don't want to miss out on any graphical improvements. Is there any visual difference between the two versions?

Screen space reflection sems to don't work in DX11 at High setting. There is no difference in visual between low and high in DX11. But in DX12 Screen space reflection on high looks almost so good as if it is in Ray Tracing a special water surface reflection looks really nice.
Next in DX12 for me trees and grass looks a little bit better.
On other side Nvidia Hairworks look more natural in DX11 as in DX12.
It looks better but I can almost guarentee you that it doesn't look that much better that you will notice it Unless it's side by side.
Oh, okay, thanks guys. I found a comparison and the only noticeable thing I saw was that in DX12 the grass and trees moved a lot more with the wind. Was that a DX11 bug or is it really only like that with DX12?
It's a lot better. You can tell from Youtube videos but yes it costs about double the amount of frames. I wouldn't play it on DX11 to be honest. Water, HBAO, shadows etc. all of it suffers a lot.

Shouldn't "cost" that many frames, they just messed things up with the latest patch. Like I said, I tried the update out when it released and it was running at a pretty solid 70/80 fps (everything on Ultra+ except RTX and no hairworks cause I think it looks ugly).

Don't listen dx12 fanboys. Apart from RT and broken on dx11 SSR high settings differences are negligible.
Although i do recommend you to install HBAO+ mod for dx11 since cdpr so conveniently removed it in 4.0. It looks far superior to ssao.

I agree. But dx12 is slightly better. Just slightly. I think they bring back hbao in the next update if I'm not mistaken
