(Oh - just bin the user settings in My Documents and the game launches and doesn't crash after you hose up all the GFX settings.)
The file to rename or delete is WRONG. The correct file to rename or delete is named:
"d3d11on12.dll" NOT "d3d11tod3d12.dll" as this latter file no longer exists at all in the Witcher 3 folders
Here are the COMPLETE steps to fix this.
To fix Witcher 3 (with Next-Gen 4.0 Patch) running in OBS 28.1.2 in DX12 mode:
1.) Navigate to the Witcher 3 folder at (default Steam and Witcher installations locations):
"C:\Program Files (x86)\Steam\steamapps\common\The Witcher 3\bin\x64_dx12"
2.) Then you need to either rename or delete "d3d11on12.dll"
3.) Restart the game and it will work then.
You're welcome!
Thanks to SaburMoto on the OBS "Windows Support" Forums who discovered this SPECIFIC file. He rocks!
Follow me @BukaTV on Twitch to see me stream this excellent game too!

I use those settings for all games and just change the drop down to whatever game im playing in the window.
https://www.youtube.com/watch?v=eBNd12Nvoag <-- recorded that with OBS with those settings
BTW. Obs is great for streaming, but for gameplay video recording better to use shadowplay from nVidia geforce experience, is less demanding
