Red mutagens - where art thou ?
I seem to recall a certain village pre-update, where one could literally farm red mutagens from ghouls. IN particular, an alghoul would spawn with the regular ghouls. After killing him, fast travel back to the starting zone and THEN back to Velen to respawn and start the whole adventure again.

I can't find this village on my recent playthru and I've explored every question mark in Velen - I think it's been disappeared. NERF ?

They've always been the rarest mutagens, for whatever reason.

Mmm, I have some good news for you, though it is in the dreaded spoiler territory...

... You can transmute mutagens from one color to another in the Blood and Wine expansion. It takes a few extra ingredients, but yeah you can go Lesser to Lesser, Normal to Normal and Greater to Greater of any color. You also need like 15 greater of each color for the mutagen upgrades so don't sell your extras!

No, not really... Keep all your mutagens, by the time you unlock Corvo Bianco you can break down all special mutagens into base mutagens, which should set you up with all the blues and most of the greens you need. The remaining blues you can transmute relatively cheaply into whatever you need :)
Plus a lot of creatures in Toussaint drop regular and even greater Mutagens (at least on NG+, didn´t play 4.0 with a blank slate yet)

Well I'm trying to find a ♥♥♥♥♥♥ captain to sail me to Skellige................. onle one (the female) has spawned.

Others are drunk, I guess ?

By that late time in the game I've long managed to find more than enough to have all Greater. That's favoring a sign/sword build.

Well it took about 25 attempts but I farmed enough wee reds to make two bigguns.

Rare indeed. I have so many greens and blues...........................
