To address your second question, weapon\armor levels exist to prevent power creep and to encourage exploration. You might think you want a level 30 sword at level 1, but if you still had to fight hundreds of level 1 through 30 opponents till you reached enemies that could actually rival your gear and present a challenge, you'd get pretty bored. Having the best gear spaced out by level also kinda forces you to experiment with some of the weapons\armors you pick up that definitely aren't the best but might be fun to mess with for a time. Maybe this ax looks fun to swing for abit instead of a steel sword, maybe this chest has 100% bleeding immunity, maybe this new silver sword has 30% chance to freeze, etc.
As to your third question, the game marks enemies with the appropriate sign for your level so you know what kind of gear is going to drop. It isn't telling you "you can't beat this" as much as its telling you "come back later." The game will reward you for jumping ahead anyway (by dropping the diagram, wep, etc) if you do beat it, but its not designed to let you equip that level 30 sword at level 1 for reasons I explained above.

Thing is, a mod to change or remove it doesn't make it better, just different. The system is there for a reason. Like AFD86 said above, the game would be a cakewalk without level reqirements for items. Is the system perfect? no, but is is the best available in the game, mods just make things easier.
I am kind of new to the game, but I have noticed just how silly the weapons/armor is too. Pretty much every mission, I find a sword or piece of armor that is "better"; but I noticed that most simply do more damage/take more damage, but generally don't have the upgrade slots of the witcher gear; so you can do more with that then with regular gear.
