You don't get it from adding a mutation, but for developing one after completing the quest chain Turn and Face the strange in Blood and Wine, given to you via a letter from Yennefer.

I suggest you do the quest as soon as you get it, because of the massive boost in abilites it gives you. Same for upgrading Corvo Bianco for awesome buffs, and taking part in the Tourney, which will give you pretty much the best crossbow in the game. All of these can be done at the start of Blood and Wine.

Something odd.
I finished this quest. I'm sure, I just read the walkthrough for that quest.
However the quest is completely missing from my list (finished, unfinished, failed). I am able to develop mutations.
I just added the achievement using SAM. I have no interest in going all the way back with a savegame
Already have the crossbow. Spent allmy money getting the grandmaster wolf gear so nothing left to complete Corvo Bianco (I did upgrade the house)

So you have the extra C menu in your Character tab? The mutation development is in there, activated with ability points + mutagen + alchemy ingredient (i think), after activating one, you get the achievement. It might've bugged if you've already done that, and still didn't get it.
As for Corvo Bianco, I left it for the end the first time I played, and was very sorry because of the bonuses. Second time playing, I upgraded it as soon as I got the quest, then got the mutations, then the crossbow, then the grandmaster set. Rest of the game is going to be a piece of cake.

Yeah, 2 AP and a mutagen afaik. Its there so the achievement was bugged for me. I'll first need to finish one more grandmaster item and then I have the set complete. After that I'll focus on Corvo Bianco. Thanks for the advice.
Btw, achievement could be bugged because I used the always fasttravel mod.

Yeah that's probably the case. There are some bugged achievements, like The Puppetier one, to make enemies kill their own with Axii, but the quest-related achievements are fine.
