As for Wild Hunt, they are able to travel between different worlds. So is Ciri. She is actually much better at it, becouse she can do it directly, while Wild Hunt relies on some universe gimmicks. As far as you accept that island as different world, it makes sense.

I don't think they are dead dead, at least in the game storyline. And I didn't read Season of Storms, I heard some things are explained there (as far as books storyline is concerned)
It is explained by Avallac'h that she fought the Hunt in the world that you visit at the end of TW1 and while travelling with Avallac'h in TW3. She freed Geralt and left him in Kaer Morhen, then ran away.
If I remember correctly, in a conversation with Yennefer in TW3 Geralt says Eredin was responsible for their amnesia. He probably did it when Ciri was saving Geralt.
He met him earlier in the books, while trying to survive in Korath Desert
