Tried Dragon Dogma as it is suppose to be about similar era - such a horrible mess. Asked for refund as it was just unplayable.
Have played Dragon Age Origins several times but never actually managed to finish it as I always lost interest halfway. It is pretty close if you looking for more mages and kings and monsters. Just not as good as W3.
The original Witcher game.
Also there are two games medieval games (Nehrim, Enderal) using the Oblivion and Skyrim engines.
Going back in time, the Drakensang games (Dark Eye, RIver Of Time) are very similar but are dated now. W3 could be viewed as a modernized Drakensang.

Darklands is 25 years old now. It is set in medieval Germany. The reference manual is almost an rpg handbook on 15thC Germany. One of the all-time classic rpg's.

Spiders 3 rpgs. Mars war logs. Bound by flame and the technomancer are to a certain degree similar, they feel like autistic versions of the witcher.
Aside from the ones already mentioned, you might try Gothic 3 and Risen. Gothic 3 is a similar sort of European medieval adventure, it has some rough edges but overall interesting free-roaming gameplay. Make sure to get the community patch as well. Risen is made by the same devs, and is overall more polished and accessible, but somewhat smaller in scope. You might wanna start with Risen, actually. Just avoid the sequels, as they aren't as good IMO.
Gothic 3, is simply put, ♥♥♥♥♥♥♥ amazing!
I'm going to try to mod the hell out of my copy of TW1 and see if that satisfies my craving lol.
Yeah, I've heard from many people that Gothic 2 was superior, but I tried it a while ago and couldn't get into it because of the primitive graphics and clunky controls. G3 and Risen would be more accessible to a modern gamer.
I still have the original ps1 copy :D maybe I should grab it out someday again.
Gothic 3 has a more fluid combat and not the stand and slash at eveything.
Its def worth it! Even if the graphics are meh, the combat systems and management systems are sweet!
