The game has native DS4 support, but you need to disable any and all xinput emulators, including Steam's own (Steam, Settings, Controller Settings, untick everything) - the last bit you are aware of exists, I gather...
And MOST IMPORTANT, you need to plug the controller in with a wire!
Even if you use an official reciever, the game will consider the controller an X1 controller due to the wireless connection.
Try a cheap USB extention cord, if you are sitting more than 1m away from your PC.
If you insist on wireless - and can manage playing with the input lag - you need DS4Windows and a mod from the nexus to get the correct button prompts on screen. Search the forum for a link, as I've never used it.
It sounds to me like you have a custom profile active in a xinput emulator, that automatic overwrite the game's own button mapping.
Disable that - and the Steam emulation - and try again.
Go into documents/The Witcher 3 and open input.settings with notepad, then you will need to search and just change the lines for your liking, that way you can rebind any button.
