There's no one source for it. Check the various alchemists and herbalists. If you can't it in Velen or the alchemist just to the northeast of Oxenfurt, try visiting Tomira in White Orchid.
You'll find the moon dust formula when you visit the hafling herbalist east of the north tip of Oxenfurt and - surprisingly enough - Oxenfurt's blacksmith.

I don't get what's your problem with Yrden here. From this comment I assume that you somehow misunderstood how it is supposed to work. Yrden doesn't stop the wraiths, it doesn't even slow them, at least the slow is not noticeable, if any. But it makes them easier to kill (provided that they remain in that circle), it also prevents some from disappearing/teleporting, etc. Since you're a higher lvl than the suggested one, you must be doing something wrong. I almost killed that wraith being underleveled (lvl 5 or so) on Death March difficulty (but gave it up for now, switching to the main story quests)
