Yes u can. Just go to closes signpost on your map. You must physically be by the signpost (in game) to fast travel. If you are at the signpost, you will notice an 'E' above it. Press 'E' and the screen changes to the map. From the map, click space ( im not sure ) and u will see whole map with regions. Pick on and click a green signpost near where you want to fast travel, and press 'E' again.

This, but instead of space my key for that is the mouse wheel. You might need to check what key you're using for that function :)

Doh I feel so stupid! I am using an xbox 360 controller btw. I bought up the map and there was a little box on the right side with some text saying "press Y to see the "World Map".
In my defense it's 4:22am...
