This mod replaces the Arachnid models with other monsters from the game, though depending on how severe your phobia is and your specific triggers, those very same monsters might still trigger you, as they have similar skeletons, and thus similar movement, if, for example, the leg movement of arachnids is a trigger for you.
Check out the included pictures on that page, those are all the examples of replacement-models, to see if they are tolerable for you.
Can't tell you if this mod works with the newest game version or not, the description in any case hasn't been updated since game version 1.21, and we're on 1.32 by now.

All that said:
The Arachnids/Arachnomorphs in this game aren't really of the huge'n'hairy variety, and I personally cannot remember any cutscenes with closeups either, but then again I'm not arachnophobic, so I wouldn't have noticed them especially if there were any.
In battle, you never really get close enough/stand still long enough to really notice details such as hairy legs on enemies, in my experience, though that in turn might stem from me being currently on my 6th playthrough, so I pretty much kill anything within seconds, provided I prepared well enough for the fight.

Yes I was hoping for something like this. These are not realistic at all.
While they may move similarly or may even sound the same, it's the visuals of a realistic spider that scares me the most and these will do just fine.

Is this game riddled with spiders? or is it really just one specific zone or DLC would you say?
Not even remotely riddled, personally I'd even say that true spiders are among the most rarely seen enemies... usually you get to see the truly phantastical/mythological beasts and monsters, and where "mundane" beasts are concerned, you most often see wolves or the occasional bear.
But there will be spiders (black & hairy) in DLC Heart of Stone, and centipedes (very big) in DLC Blood and Wine.
But I'm 100% sure you won't encounter any spider during main quests of any DLC, unless you wander into the wild.
This little lady was never strong enough to slain a giant spider haha.
Just small ones, like the ones found in Diablo and Grim dawn.

But, Jam, PLEASE do play the addons too, especially Blood & Wine has such a magnificient new realm to discover and such a beautifully-written main story, one of the most memorable side stories out of any game I've ever played (Canary is all I'll say), and this addon is actually one of the exceedingly rare examples of a grand adventure being closed off perfectly.

No need to reply to this if this makes you feel uncomfortable or anything... just think the world would be a nicer place if ppl, for a change, just speak up when they think they can help.
I agree, it's by far my favorite DLC of all time.
Well, I'd say get the GotY, but I also think this is the greatest game in history so I'm a bit biased.
The only things that "pop up" are the two expansion DLCs, and that will basically just tell you "Thank you for purchasing" or something like that once you get past the tutorial area. The in-game journal will tell you a recommended level for all the quests (including the DLCs), so there will be a natural way to progress through the game. The second expansion, Blood and Wine, is for story reasons recommended to play after the main quest.
There's also a bunch of smaller free DLCs, but I don't think you'll even notice which ones those were since they're just quietly added to the world.
And just a suggestion: If you buy the GotY, you might be able to disable Hearts of Stone while going through the main game so that you don't accidentally run into the arachnomorphs while exploring the world, and then maybe enable it when you feel ready to play it. Either that or look up at what areas they spawn, I'm sure it's on a wiki somewhere.
