I fixed it by verifying game integrity, it restored 3 files.
I have noticed that when I manage to put away my sword right before a cut scene, abandoned village stuff, that when the cut scene finishes of villagers returning, I have my hand at my side like I'm still holding a sword. Until I fast travel it stays like that.
I take it fast traveling or jumping to another different map location (Novigrad to White Orchard) doesn't clear the graphics?
Edit to add: Nm you got it fixed :)
Now im not sure but i believe the problem is caused by multiple save files. Like i said, this mostly affected the witcher swords and in my completed game i had only witcher swords in my cache. I guess something went wrong with recovering the skins for the second playthrough that wasnt ng+, but verifying game integrity did solve everything, including the npc swords.
Still, thanks for taking the time to answer
