His mother belongs to a druid circle in Mayena so it's presumed Geralt was also born there before he was given up to the Wolf school.. which would make him Temerian.
But his official place of birth has not been stated in the books.

Geralt originally went with a Nilfgaardian name Geralt Roger Eric du Haute-Bellegarde which Vesemir said sounded silly and pretentious.. so he went with a much easier and unofficial of Rivia title instead.. which he also adopted a Rivian accent to make sound more authentic.
He was officially knighted sometime later by Queen Lyve of Lyria as a reward.. making his of Rivia title actually official.
Bigger question is how does he avoid cutting off his ears every time he pulls the sword out....
Well guess you'll have to either find a helmet mod or make one.. probably better off playing Skyrim or something if you want to RP a helm wearing nordling though.. Witcher games are quite heavily story focused so.. it's not the best game for role playing.
And Yennefer, as a true woman, loves nothing but true men.
And vice versa...
