I'm sure you have already picked up several task from notice boards that are far beyond your level...No point in going after those untill you are ready.
Or prepare to grind ingredients for those silly potions and bombs of which I don't know where to find them or when to use them or even be able to judge how effective they are. If you even get a chance to use them during the clunky combat. Or maybe use the spells which interrupt the flow of the game as you try to choose from the near identical icons and then have it do almost nothing to help you but waste time and effort.
No, I'm not impressed with the mechanics of this beautiful looking story rich game.
Bestiary and descriptions are your friend. Research, Witcher. Moondust and Dimeterium for things like blocking the abilities of foglets, werewolves, vampires, and wraiths. Elementals too.
Handy stuff, to be sure.
I suggest never use the manual targeting system in the game its clunky as hell. Better install some mods to improve the control of this game.
Tip: Crossbow do auto-aim
The game is actually forgiving even in higher difficulty, you got plenty pots and skill to regain your health in the game utilize them wisely.
Also some monster have aggro zone, you can easily leave there aggro zone except for range ai and scripted boss if you think the Ai is too difficult for you.

Also listen and understand the story it will give you some hints what monster are you facing.
Also buy everything related to decoction, oil, witcher gear and bomb, it will make the game much easier if you got all of them.
You can't be serious with this. There are 4 signs. Yeah, they look similar, but only in that they're all a species of triangle. Took me maybe an hour of gameplay to sort them out. Also, there are pop-up tips when you choose one that tells you exactly what they do. The Bestiary tells you what signs/bombs/oils are weaknesses to different monsters. Spend just a second reading and you'd know this. You can even pause mid-battle to check on it, arm the proper oil/sign/bomb, then dive back in. The combat is a bit clunky, but it's not horrible like it was in the first Witcher game. So far, Witcher 3 has been an absolute pleasure.
You can explore the Velen area on your own from level 6 and above. For example follow this map which has average enemy levels marked.

But you get almost no xp for killing random monsters. You should be doing quests for xp.
At level 4, did you clear up all the ? marks in tutorial area of White Orchard? Not only gives finsihing up the map you some xp but you also get gold and may find recipes.
About battles:
Lower difficulty in game options until you find the fights easy enough. In the main menu Options >> Gameplay >> Difficulty >> just The Story. Also recommend checking following settings there: Enemy Upscaling OFF, Automatic camera centering ON, Movement Response ALTERNATIVE, Manual sword drawing OFF,
You can easily die even at level 30 if you run into group of wolves or wild dogs. Try to position yourself so you can fight one enemy at time and not get overrun from behind.
There are several ways you can fight, at least 3 different ability trees, multiple equipment sets etcetc. Experiment and try to find something that fits YOU best, not something the game suggests or other players use..
I will explain my own fighting style, to me it feels best and I have finished the game on highest difficulty without problems using this.
- abilities you need, first invest and equip general skill Gourmet (you need Blood and Wine expansion installed for that) which makes you regenerate a lot HP both in the battle and outside
- next invest all the points in Fast Attack under combat tree
- in the quick access screen make sure you have Quen sign picked
- at the start of battle cast Quen using Q key. Quen shield will make enemies first attack to miss
- learn to roll. You roll by holding down SPACE and pressing directional keys WASD. For example to roll TOWARD enemy you hold space and press W. Rolling allows you to quickly get close to enemy or escape when being enemy starts to hit back. You will not take damage while rolling, enemy will miss all the ranged attacks too. When possible, roll toward enemy so you end up BEHIND them.
- now you hopefully got melee range, just click Left Mouse Button couple times to hit the enemy
- remember, your Quen shield will block first attack from enemy
- before enemy can hit you back, roll backward using space + s
- recast Quen
- roll toward enemy, and repeat your attacks
- this way you should be able to avoid most damage
Quen exploding shield & active shield -> convert damage to health, can heal from 20% to 100% easily, exploding shield give ministun
Yrden tier 1 (intensify? i forgot the name) & tier 2 (magic trap) -> have ministun, flying enemy can't fly, enemy slowed down, can clear group of weak enemy without you swinging sword, so damn useful
Warning : game will be very easy after this point
tips :
- never combo more than 3 hits
- sun & stars ability give regeneration , very good at harder & hardest difficulty, but useless later
- free meat & money by killing chicken , cow, goose, pig, and goat, use igni, do it at night so guard wont attack you
- be careful to not over kill cows at white orchard, or just dont do it
- do not repair your weapon, weapon wont break at 0%
ps : igni isn't my playstyle, you're witcher not an archmage for allgod sake
Monsters that are 5+ levels above you take much less damage per hit and do much more damage to you.
Oh yea, and even if it doesn't seem like something you'll like (I didn't at first), give Gwent a chance. It turns out to be a fun game and a bit of a collecting obsession. You'll meet a lot of people in game that want to play a few rounds.
