Super old post I know but given the transformation in the cinematic shared above it would be safe to say she though she is a higher vampire she's actually a bruxa, not a "True" higher vampire. Bruxa need blood to live despite being considered higher vampires due to their incredible level of sentience.
When considering the context of "You didn't think I was doing this out of the good of my own heart, did you?" to me implies that she is doing it because she needs it to live as a bruxa; the cinematic showing her as a beautiful, youthful woman that turns into a featureless, though shapely, vaguely skin coloured - closer to grey creature, tied into the fact that you can kill a bruxa and Orianna is killed in the cinematic, whereas you cannot completely kill Regis or Dettlaff without the help of another vampire as they are "True" Higher Vampires and only one of their kin can kill them.
Furthermore if she were a higher vampire and had an addiction, Regis likely would not celebrate her as he does. Regis is a recovering addict yes, however he does his best to actively help humans, instead of harming them, I don't think he would stand by Orianna so willingly using children for blood at the point we see him in B&W.
There's gotta be a mod around. I know there's one that's super poorly done in terms of voice acting, the animation is good, but you save her which is a real WTF moment. Heeeeellll naw!!!
Your employer sucks your blood to give you a chance to pay rent and employers don't treat you better than how Orianna treats her kids. She is a hero, the other humans have abandoned those kids, not even Anna Henrietta noticed them, and she needs that blood to survive as she is a Bruxa. Those kids are safe and the opposite of harmed. It sucks that we cannot choose how Geralt will react, Geralt even makes buddies out of rapists and murdering viking plunderers. You can even choose to save Morkvarg or let a woman in Crow's Perch get gang raped (exclamation mark event).
Because it's not prostitution, it's more akin to labor. Except the average labor worker pays so much more than occasional mililiters of blood. Your employer is your blood-sucker, but they're generally no kindly saints for the blood they suck. Workers pay in health, frustrations, stress and years worth of work to save money, pay rent and support their families.
