Quest: Feet as cold as Ice *very minor spoiler*

Hello, I am just doing the second expansion of the game and ran into this quest. Upon hitting it up on youtube to check something I was confused because the person giving you the quest apparently had undergone a major change. Some people have a bit more bulky brunette woman as the quest giver while others (me included) have a smaller blonde woman. The dialogue appears to be the same but the character was completely changed, even though both models are very unique compared to other npcs.

Anyone know the reason for this? Could not find anything about it on the wiki.

Looks like people are using mods that change npc's appearances. That's why they look different.

Are you sure its a mod? Why? I thought it was an old patch. Why would you mod the NPC of a sidequest?
