Game too glitchy !

I finished this game twice already; in PS4 and my first playthrough in PC.
I decided to start a new game yesterday but it was too glitchy, something that never happened before when I played it in PS4 and PC ( my first time ).

The first glitch happened when I go to Vizina to meet the emperor and I have to dress in black. The dresses where not there so after going back and forth to repeat from the last save, I decided to get the black dress via console command and I could finish this part. Resolved.

The second glitch is more frustrating. After I did meet with the Baron, he let me inspect Ana and Tamara's room. The talisman never showed up and the quest does not progress without it. After going back and forth several times, deleting and restarting the quest, I gave it the last try : went to Tamara's room to retrieve the key, the incense and the doll. Then I went to the basement and interacted with the Altar, however, there is supposed to be a noted but it was not there. I went back up to Ana's room, inspected everything and finally followed the fume down the stairs to retrieve the Talisman but it was not there.

So at this point, I am very frustrating because I never had those two problems before, not even in my PS4. I have played this game for more than 450 hours combined and I only faced 1 glitch long ago and it was not any of the above.

If someone knows how to fix this, please help. Something happened lately with this game. I do not know if it is the last patch or something but it was not like that at all. This game has been knows as the Jewel of the Games and still is in the back of my mind but with those glitches ( and I guess more down the road ) I just simply are completely atonished and very frustrated about it.

I would appreciate any help if any. My rig is not the problem : Nvidia 1080Ti, i7-8700, Z370-e gaming motherboard, 16gb RAM and 1tb SSD 970 EVO. I am also playing Fallout4, Skyrim and Dark Souls 3 and not a single problem at all.

I'm on my second playthrough now and havn't had either of those. I've only experienced poor design issues and minor annoying bugs.

Using mods?
Maybe a corruption issue? Verify Cache, start a new game for verification, and/or reinstall?

the conversations do not keep up with the 'lips' of the people....draggin behind....annoying....

Try re-loading the game. I have zero issues since I started Witcher 3 GOTY. Sounds like it loaded improperly.
