Same as with a sword fight.
Block (i got it binded to Right Mouse Button) an incoming fast hit to get an advantage and then make 2-3 hits of yours.
Dodge (i got it binded to Alt) an incoming strong hit and make some fast hits of yours from your opponent's flank.
You might also try to do strong hits (i got them binded to Shift-Right Mouse Button) against blocks, but i'm not sure they are very effective.
Nothing hard, i believe.

If the opponent stun you, you are unable to make any defensive move(dodge, block), and can beat you with 2~3 fast attacks.
This happens often at the start of the fight where he leap and hit you, pushes you into a corner and finish you with the fast attacks.

Forced fist fights in main story are just the worst, epecially on DM. You can't even panic-spam-attack through them... You can of couse hold the block button (RMB), but you won't get any advantage from it. What I usually do is when the fight starts I immediately dodge back (default is Alt, I think. Mine is bount to Mouse5), to get a bit of room and wait for a strong attack, then counter (quick tap of RMB) to stun the opponent, then fast attack him 2-3 times and dodge back again before he atatcks. Always keep a distance, that will force your opponent to use a strong attack, which is so much easier to counter or block.

So in short: Dodge, counter, hit, hit, hit, dodge, counter, hit, hit, hit, etc. (Keyboard: Alt, RMB, LMB, LMB, LMB)

boxing kinda sucks in tw3 but anyway. make sure to keep your guard up 24/7. then dogde and punch exposed side or back. dont barrage attacks or you'll get countered and dazed. unless you've lvl advantage ofc. and if enemy's lvl advantage be especially careful. but evne then if you can execute flawlessly ad nauseum you'll surely win there's no weapon wear at least ;)

what difficulty lvl btw?

Yeah, especially when it's a "lose and you die" type of match, not "lose and you're told you're a wimp, then continue on with the story like nothing happened". My last game was NG+DM and I spent about half an hour with that stupid racist half-elf.

best parts're those 2v1 or 3v1 fights :) except for camera ofc.

If you're referring to my half an hour with the half-elf, then I didn't insist on doing things wrong and doing everything the same way every time, I spent that time "learning to play the f*ing game" as you so eloquently put it, changing my approach until I found the strategy that works for me and perfected it, as I explained in my first post. That approach works for me every time, now that I "learned to f*ing play". NG+DM doesn't exactly leave room for mistakes, one wrong move and you have to start from the beginning. You don't "git gud" in one try.

At least some of those fights don't function as actual fist fights, so you can use potions and signs.

The fist fights really aren't all that hard once you learn the routine. Keep a bit of distance and keep your block up. Your opponent will swing, so dodge and get behind them. Punch once or twice and get away. Rinse and repeat. If you can get them backed into a corner, them you can just go to town on them. Of course, they can do the same to you, so watch your surroundings!
The fist fights are some of the easier parts of the game. Those fist fight sidequests were the only quests in the game i could perfectly do underlevelled.

the fist fights were the easiest thing in the game for me. The enemies only ever throw 2 different attacks a quick jab like punch and a slower hook power type punch. Thats it. thats the extent of their ai. Can they even throw a kick? NOt sure as i dont remember a single time one of them threw a kick.

This makes them perfect canditates to parry there punches and counterattacking. I rarely even get hit in fist fights. When i do its only when fist fighiting 3. And even then its only because of 1 thing i can effectively change so it wont happen again.

Which is rarely while going to hit an opponent and geralt thows a kick instead of a punch because its slower one of the other enemies runs in and hits me before i can dodge.

Which i effectively negate by simply choosing the time to thow a attack when the other enemies arent close enough to run in before the animation ends. So ill have time to dodge.

The length of time of an attack animation is the only reason i ever get hit in a fist fight.

And it only happens when fighting multiple. and Even still its only happens rarely.

Far to rare for me to actually lose a fist fight.

All you can really do is mash the backwards dodge at the beginning of the fight to give you some room and then throw quick 1-2 fast punches whenever the enemy is winding up. Don't try to block, they will just stun you.

If you are still frustrated then turn the difficulty down, it's just a game.

The fistfights are terrible, they've been terrible since day one, some people like them but for most it's an unbalanced chore.

DM boxing is just bullcrap, when you have hp about 3-4 hits and should land something like 50, but it going to be a problem with lvl increasing, things should be more or less fine untill lvl 13 or something, and above there is almost no 'win or die' forced story-fights

@welfd

There's nothing wrong with fist fight.

The key is counterattack - very useful in melee, not only in fistfight. After a successful counterattack Geralt gets some free hits. So you can manage to beat opponents several levels above - with the red skull:
